In order to simplify your first visit, we have the necessary paperwork available to you in a simple online format. This info will be sent to our office to make your initial visit check-in quick and easy. Just click the link below and you will be directed to the secure form.
NOTE: If the patient is 12 years or younger make sure to select “YES” when asked, “Is this condition associated with a child development concern or condition?”
What to Expect
During your first visit, a comprehensive evaluation will be completed. In addition to reviewing your health history, we will choose specific tests to determine whether chiropractic care is safe for your presenting condition. If it is determined you are a candidate for care, your treatment will start immediately
What to Bring
Our chiropractic office is located in Tempe, AZ. If this is the first time you’ve been to our office, please arrive 15 minutes early. Please bring your ID and insurance card. It is best to wear clothes that do not restrict movement or mobility, something loose-fitting and comfortable like sweatpants, basketball shorts, running shorts, yoga pants or leggings are best.
Payment Options
We accept cash, personal checks, and major credit cards including Visa, Mastercard, and Discover. We also accept health savings account (HSA), flexible spending account (FSA), and health reimbursement account (HRA) payments toward care.
Insurance Accepted
We accept some insurance plans, and have affordable cash plans available for those without coverage or high deductible plans. We submit claims for the plans we are in network with, and are happy to offer a super bill for those that have out of network plans. We understand that maximizing your insurance benefit can be important and if checking your benefits would be helpful to you, just ask!
Cancellation Policy
We reserve time in our day specifically for you. Missed appointments without proper notification can result in an additional fee. We understand that emergencies sometimes happen so we ask, whenever possible, that you kindly give us 24-48 hours notice if you are unable to keep your scheduled appointment. This advanced notice will allow us to offer your appointment to another patient. We appreciate your consideration.
